  3. hi Erica, I've been working in 2016-2021 in LGU and services of assistance regarding Birth Certificates Indeed records are needed please do not apply for late registration it will make double entry and will make it worst and their is MULTA.
    first IF only letter is the problem the case in called petition for correction of clerical error in the certificate of live birth
    it is not per letter the amount you pay it was per case ok, to avoid scammers
    regarding the school records try to ask the Regional & Division Offices of deped evey records of schools are forwarded to their offices.
    Baptismal certificate
    Valid ID's
    Marriage contract
    they are need as what name is used by the individual
    in the birth place of your mother is where you will fill.
    some cities accept other cities notary but some are not so better make notary in the same LGU ok
    and last find LCRO local civil registry Office in that Local gov. unit or city

Get a lawyer to help you with the name change: notarized, showing she's one and the same; get the birth cert from NSO, and then get a lawyer to notarize the birth cert, and if you can, go to Foreign affairs to put a ribbon on the birth cert. I guess you need it to be certified. I'm not sure if it's still the same process. But it's doable. Goodluck! I don't think you need the Elementary school records.

  5. Pero kung wala po sila birth cert, ang pede gawin, katulad ng sa late mother ko po nag pa affidavit po sya na nasunog ang munisipyo nila at wala ng record ang lgu ng new Washington aklan tas pede nyo po I late registry ang birth cert ni nanay although dami requirements tyaga lng po tlga

  6. Kung meron po kayo pababago sa birth certificate ng mother at father nyo sa local civil registry po kayo ng kung saan lugar po nakatira si nanay at tatay, kung saan po sila butante

  8. Regarding for change name you just go to lawyer as for me after graduating to college my name is very long. I only use my 3rd name which is Florine and when my birthcertificate came its Maria Rica Florine just my mom had a power of attorney to settle.

  9. hello there, if the elementary school is not there anymore, pwede mag-inquire sa district where the elem school belongs, the record of elem school is important and also the high school record too in changing of name to prove if they are the same in what name she is bringing up to now

  10. Lola’s name in the birth certificate is different from my school records (first name & last name). First name was longer & last name was changed because my father became a naturalized Filipino citizen. What my parents did was had our family physician wrote a letter attesting that the person is the same -that in the school records, the first name is a shorter version of that in the birth certificate & the last name was changed due to the naturalization of the father (changing his Chinese last name to a Filipino surname taken from his mother). Why the change? At that time, it was easier to do business with a Filipino surname. The letter was notarized with a seal & it served as an Affidavit. The naturalization papers of my father was also needed as proof for the change of the last name. When I migrated to the States, the affidavit from our family physician was adequate to show that the name on my passport is the same as the person on my birth certificate. I also needed to include that affidavit when I applied for naturalization for U.S. citizenship. In my case, it was easy. Hope your mom’s will also be easy. I did not need a lawyer.

I think he will be an athlete (picked the ball twice).😀 Dana had a great time with cousins. It was a good decision to celebrate his birthday there.   11. My mother has the problem and you just have to get her NBI and other paperwork follow the correct spelling.

    I am seeing your journey as a married couple. I have been married to my husband for 32 years now. I have to move to the USA in 1992. My parents stayed with me for at least 7 years. I am the oldest of four children and all my family is in the Philippines. Both my Mom and Dad passed away at 69 and 63 years old. Love your family and at same time Ricky is now your home.

Hi! my ideal husband Ricky & Wife Erica's short hometown Christmas visits but happy & meaningful coz' it's first time Jios 1st birthday done in the Philippines at Jollibee,Everybody was so happy young & old relatives joined the parlor games,Erica cried at their departure going back to S.Korea's family home,maybe she cried & missed her family. Both Ricky & Erica's family are eagerly & happy to meet especially their mga apo's. Have a Merry Christmas & Prosperous New Year's to everyone!!!***

  14. Erica my name is mispelled as well i only find it out when i was about to take the board exam so what did went to a lawyer and made a documents that the name in my school record and my biryh certificate Is the same person .

  17. Hi Erica, I went through the same thing about the name in birth certificate when I was processing my papers for US bound I took it to the court ( local municipal court ) that was the advice back then idk now, I did not used a lawyer at all

  20. Hi erica and ricky regarding sa birthcertificate ng mom mo siguro same kami ng problem sa akin naman is yong family name ko mali ang spelling at di ako maka graduate ng college pag di naayos. Ang ginawa ng mom ko pumunta siya sa LCR kung saan ako naka registered tas bigyan ka nila ng mga requirements tas pumunta din siya pao office. Kasi i court hearing pa yan at ipapublish sa newspaper. It takes 6 mos sa akin at ang nagastos namin noon is 4k way back 2003. Sa kapatid ko naman na babae gender din ang mali sa kanya instead female male ang nakalagay pero naayos din same kami ng scenario hindi din siya maka graduate ng nursing kasi mali din ang gender niya. Pero ngayon hindi na mahirap mag change ng name, family name etc at hindi na mahal gaya sa amin noon. I hope makatulong ito sa mom mo. Basta unang gagawin ng mom mo pumunta siya sa LCR (local civil registrar office) kung saan siya naka registered sa munisipyo di mawawala ang record niya doon if in case walang hindi siya naka registered pwede siya kumuha ng late registration may fill upon siya doon na paper about sa parents family name niya and maghintay siya ng not more than 1 month bago marelease ang b-certificate niya.

  23. In my case, I've been using only one name from elementary until college because I was not aware that I have 2 names so before graduating from college I went to see a lawyer. It was an easy procedure back then. It was a good decision because I didn't had any problem when I went abroad for work. Good luck!

  24. Yong pinsan ko mayrong problema Rin sa birth certificate niya kaya di makuha Ng passport pero nagawan Namin Ng paraan pumunta kami sa kaibigan Kong attorney at humingi Ng affidavit of attorney regarding sa error last name niya at spelling Ng middle name cont.

My husband had a problem also with his name before, he just went to a lawyer and brought his birth certificate and the id that he's currently using then from there they issued a letter saying that this person whose name in the birth cert is so and so is the same person with the name of so and so, then he changed all his ids from ising that letter from the lawyer.
